This page allows Licensed Nursing Home Administrators to register for the new Online Self Service License Maintenance and Renewal tools.

To register, you will need your myBELTSS Self Service Introduction letter from the State Of Ohio Board of Executives of Long Term Services and Supports (BELTSS). This letter provides your Username and your Temporary Password that you will need to register for this website.

  1. Enter the User Id and Temporary Password from your MyBeltss Introduction letter.
  2. Create a new password that conforms to the password rules.
  3. Enter your Personal Email Address

After you complete this page, you will receive an email at the email address you provided. This email will contain an email verification code. You will need to provide this code on the next page, to prove that you have received the email.

Password Rules

  • The new Password must be at least 8 characters long.
  • The new Password must contain at least 1 UPPER CASE character (A-Z).
  • The new Password must contain at least 1 lower case character (a-z).
  • The new Password must contain at least 1 numeric digit (0-9).
  • The new Password must contain at least one special character from this list. !@#$%^&*()
  • The new Password must be different from the temporary Password.
  • The Confirm Password must match the New Password.
